1. In a saucepan, combine egg yolks, sugar, and milk over medium heat. Stir constantly until the mixture boils.
2. Boil gently for one minute, then remove from heat and allow to cool slightly, then chill in the refrigerator for one hour.
3. In another bowl, combine the heavy cream and vanilla extract. Whip until stiff peaks form. 
4. Beat the mascarpone into the yolk mixture until smooth, then fold in the whipped cream.
5. Dip each ladyfinger into the coffee (and rum if using), and layer them in a dish. Spread a layer of the mascarpone cream over the ladyfingers. Repeat layers, ending with the cream.
